Mishaps 05 SEP 41 A/C: PBY-2 pby Location: NAS Pensacola, Florida Strike: Yes BUNO: 0490 CAUSE: Easy Bay, Pensacola - Test flight - Engine failed in flight approx. thirty-five minutes after leaving base on test flight, resulting in forced landing. The failure was accompanied by severe momentary vibration as a sudden stoppage occurred. (see accident card 19 Aug 1941, same pilot). Crew Ok: Pilot LT Charles Tighe USCG, Ens J. F. Mishaps 22 JAN 47 A/C: PBY-5A pby Location: NAS Pensacola, Florida Strike: Yes BUNO: 08204 CAUSE: Night dual instruction Pensacola Bay. Plane crashed in south central portion of Pensacola Bay, 200-yds north of USCG Station on Santa Rosa Isl, while making a power on night landing. The instructor at an altitude of 100-ft over north portion of Santa Rosa Island, took over as pilot and made a shallow left turn, at the same time lowering the nose and increasing his air speed to approximately 80-kts. At night power landing attitude was then attained. The altimeter read below zero, when port wing went down abruptly and pilot applied power. Before power was effective, port wing float hit and skipped the first impact. At the second impact the port wing dug in causing the nose to swerved to the left and dig-in, and flipped the plane over on its back. "Strike" Crew: Pilot Lt(jg) Albert W.
